On or about February 24, 2010, and at times prior thereto, at the facility
located at 123
Commercial Ave., Lowell, Arkansas,  the seat belt on the Mitsubishi
powered industrial
truck was not functional, exposing employees to the hazards of being
thrown from or
crushed by the vehicle.
One feasible and acceptable abatement method to correct this hazard is to
ensure that
employees are able to buckle their seat belts each time they operate a
powered industrial
truck as required by the ANSI/ASME B56.1 "Safety Standard for Low Lift and
High Lift
Trucks", Article 5.3.19 which states:  "An active operator protection
device
or system,
when provided, shall be used".
