Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: (a)On or about 02/08/10 - Building #130, Chemical Process Area, Line #1 and Line #2, hooks used to lift and hold shop fabricated lifting devices were not removed from service for repair or replacement after factory installed safety latches was broken and missing. Among other methods, one feasible and acceptable method to correct this hazard would be to comply with the requirements of the American Society of Mechanical Engineers Standard ASME B30.10-2005, Hooks, Section 10-1.1.2.1.3(c)(5).

Among other methods, one feasible and acceptable method to correct this hazard would be to comply with the requirements of the American Society of Mechanical Engineers Standard ASME B30.20-2010, Below-the-Hook Lifting Devices, Section 20-1.3.9(b)(6).
