Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to falling loads and/or struck by hazards:
(a)Facility, 945 Vista Park Drive, Pittsburgh, PA:  On or about 05/20/10
the employer did
not establish a maintenance program to ensure that conveyor components are
maintained in a
safe condition.  The accordion/flex conveyor system used to unload stock
had the following
deficiencies: Broken support leg, missing cross member support, 10 missing
caster wheels,
missing bolts that secure accordion sections to frame.  The broken leg was
supported by a pallet
and a 2 x 4 was used for additional support.  These conditions exposed
employees to falling
loads and struck by hazards.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard is
to establish a maintenance program to ensure that conveyor components are
maintained in a
condition that does not constitute a hazard to personnel.
