employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing/amputation hazard from unintended activation:
(a)Facility, 314 Deveaux Street, Carrolltown, PA:  On or about 06/07/10
the foot pedal on
the Chicago 36 ton mechanical press brake, serial #L99537, was not guarded
to protect the
operator from unintended activation.
Among other methods, one feasible and acceptable method to correct this
hazard is to guard the
foot pedal.
