(a)  The process hazard analysis did not identify, evaluate or control the
hazard
of flammable premix being transferred through a pipe which contained a
valve in
an area which was not considered a hazardous (classified) location, on or
about
December 1, 2010.
(b)  The process hazard analysis did not identify, evaluate or control the
hazard
of removing one pressure relief device from the dimethylamine tank and not
controlling the position of the selector valve which leads to the pressure
relief
devices, on or about October 20, 2010.
