Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: the employee was exposed to a finger crushing and amputation hazard from inadvertent machine activation. a) Trane Area - The foot treadle on an Amada RG-35 press brake was not completely guarded to prevent unintended operation from falling objects or accidental stepping on the pedal as observed on or about 01-11-11. ABATEMENT CERTIFICATION REQUIRED Among other methods of abatement, on feasible and acceptable method to correct the hazard is to ensure guards on the foot treadle are kept in proper adjustment to fully cover the treadle.nt
