Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to: An equipment failure when outriggers were not used while
operating Altec AO
300 mobile lift.
a) Route 15 and Hospital Drive, Lewisburg PA - Employees used the Altec AO
300
while the side stabilizer was not set during operation as specified in the
manufactures
manual, observed on or about 03/03/11.
ABATEMENT NOTE: Among others, one feasible and acceptable method to abate
this hazard
is to follow the manufactures safety instructions to us outriggers when
equipped while in
operation.
