that were causing or likely to cause death or serious physical harm to

On or about May 4, 2010, and at times prior thereto, throughout the
Warehouse Area,
employees were exposed to the hazard of being struck by metal racks and
stored products in
the event of a collapse. The employer failed to ensure all metal rack
columns were anchored
to the floor.
Some feasible abatement methods include but are not limited to the
following:
1. Column Base Plates and Anchors: All rack columns should be anchored to
the floor with
anchor bolts capable of resisting the forces caused by the horizontal and
vertical loads on the
rack, as recommended by Rack Manufacturers Institute "Specification for
Design,
Testing,
and Utilization of Industrial Steel Storage Racks." 2002 Edition (such as
applicable Section
1.4.7) and ANSI MH16.1:2008.
