from elevated heights of oil-well service rig derrick:
On or about May 4, 2010 and times prior to, at HALO Energy Rig #112, an
employee
worked from the tubing board where an emergency escape device was not
installed and
available for use. The employee working from the tubing board was exposed
to a potential
fall or severe burns associated with, and in the event of a fire or
explosion at the oil-well
service rig platform.
One feasible and acceptable abatement method to correct this hazard would
be to ensure that
an auxiliary means of escape is installed and ready for use prior to
personnel working in the
derrick in accordance with Recommended Practice for Occupational Safety
for
Oil and Gas
Well Drilling and Servicing Operations- API 54, Third Edition, August
1999, Section
6.10.1, Auxiliary Escape: "On all land rigs, the derrick or mast shall
have an auxiliary
means of escape installed prior to personnel working in the derrick. The
auxiliary escape
route should use a specially rigged and securely anchored escape line
attached to the derrick
or mast so as to provide a ready and convenient means of escape from the
derrickman's
working platform."'s

struck by and crushing hazards:
On or about May 4, 2010 and times prior to, at HALO Energy Rig #112,
employees
working at rig #112, were exposed to potential struck by and crushing
hazards associated
with the possible collapse of the derrick due to the continued use of a
traveling block running
wire-rope line rubbing against and compromising a critical spreader-arm of
the derrick
locking-pins.
Among other methods, feasible and acceptable means to correct this hazard
are:
1.Ensure the Rig Operator inspects the rigging involved each time the
derrick is
erected and does not initiate rig operations until questionable critical
components are secured and not compromised.
2.Ensure the Rig Operator visually inspects and corrects the routing of the
derricks running wire-rope line and does not continue rig operations when
the
integrity of the derrick locking area components are compromised in
accordance with:  API Recommended Practice 54, Third Edition, August
1999., Subsection 6.3.2,: Well operations involving the rig shall not be
commenced until the rig is rigged up in a safe manner.
