Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing, or likely to cause, death or serious physical harm to employees in that employees were exposed to struck-by hazards: Southwest area of the workplace, Unit #1 & #2 Compressed Natural Gas (CNG) equipment pressure devices: On or about March 12, 2010, and other times prior thereto, pressure gauge devices installed at heights of 33 to 63 inches above the floor and used by employees to monitor CNG system pressures were not maintained in proper operating condition, exposing employees to struck-by hazards. The maintenance of the following pressure gauges was required by Item 4-16, Maintenance of the NFPA 52 CNG Vehicular Fuel Systems Code: a.Unit #1 Kimray Gas Regulator pressure gauge: gauge indicator needle was detached from the gauge rotating shaft, b.Unit #1 Compressor Oil Filter Inlet pressure gauge: gauge glass cover was damaged, c.Unit #1 Compressor Oil Filter Inlet pressure gauge: gauge pressure indicator needle was missing, d.Unit #1 Compressor Oil Filter Outlet pressure gauge: gauge glass cover was damaged, e.Unit #1 First Stage pressure gauge: gauge glass cover was damaged, f.Unit #1 Third Stage pressure gauge: gauge glass cover was damaged, g.Unit #1 Third Stage pressure gauge: liquid filled gauge was partially empty of fluid, h.Unit #1 Fourth Stage pressure gauge: gauge glass cover was damaged, i.Unit #1 Fourth Stage pressure gauge: liquid filled gauge was partially empty of fluid, j.Unit #2 First Stage pressure gauge: gauge glass cover was damaged, k.Unit #2 First Stage pressure gauge: liquid filled gauge was partially empty of fluid, l.Unit #2 Fourth Stage pressure gauge: gauge glass cover was damaged, m.Unit #2 Fourth Stage pressure gauge: liquid filled gauge was partially empty of fluid,n.Unit #2 Kimray Gas Regulator pressure gauge: gauge indicator needle was detached from the gauge rotating shaft, o.Unit #2 Compressor Oil Filter Inlet pressure gauge: gauge glass cover was damaged, p.Buffer Pressure Control gauge for the Fuel Pressure Control Panel: gauge glass cover was damaged, q.Buffer Pressure Control gauge for the Fuel Pressure Control Panel: gauge contained a deformed indicator needle, r.Compressor Discharge gauge for the Fuel Pressure Control Panel: gauge indicator needle was detached from the gauge rotating shaft, s.Vehicle Pressure gauge for the Fuel Pressure Control Panel: gauge glass cover was damaged, t.Vehicle Pressure gauge for the Fuel Pressure Control Panel: liquid filled gauge was partially empty of fluid. Among other methods, one feasible and acceptable abatement method to correct this hazard is to replace all defective pressure gauges with manufacturer's recommended replacement gauges.
