Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a struck-by hazard:
(a)On or about 02/23/10 - Coke Plant, employees were exposed to a struck-by
hazard in that employees as pedestrians and as vehicle operators were not
provided any safety instructions relative to vehicular traffic.
Among other methods, one feasible and acceptable method to correct this
hazard is by providing
pedestrian walkways and limited access points to the job site or work
zone. Work zones should
be designed based on the type of equipment being used. Provide signs
within the work zones to
guide workers to walkways. Signs should be provided to keep vehicular
traffic at a minimum
speed and vehicles should be forced to stop in blind areas and
intersections.
(b)On or about 02/23/10 - Coal Dump Hammer Mill, employees were exposed to
a struck-by hazard while walking under a conveyor that was 40 feet above
the
ground that dumps coal without any barriers or guards.
Among other methods, one feasible and acceptable method to correct this
hazard is to follow the
American Society of Mechanical Engineers (ASME) Standard B20.1-2009
"Safety Standard for
Conveyors and Related Equipment" in Section 5.9.2(d): "Guarding by
Location or Position"
requires the use of spill guards, pan guards or the equivalent if material
can fall off the conveyor
and endanger personnel."
