Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to: Fall hazards while performing work from a rough terrain
forklift's work
platform attachment.
On or about 10/04/10 at Larry Walker Blvd, Perry, GA: An employee was
exposed to fall
hazards, while performing work from a rough terrain forklift's work
platform attachment. The
employer failed to ensure that the forklift's work platform attachment met
the specifications of
ANSI B56.6-1992. The attachment was not approved by the manufacturer
(JCB), and its width
exceeded the maximum allowable width per the ANSI standard.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THESE HAZARDS IS:
1. The employer shall gain approval from the forklift manufacturer (JCB)
prior to mounting
the platform attachment, and shall ensure that the work platform meets all
design specifications
referenced in ANSI 56.6-1992, Safety Standard for Rough Terrain Forklift
Trucks.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included with
your certification.  This documentation may include, but is not limited
to, evidence of the
purchase or repair of the equipment, photographic or video evidence of
abatement, or other
written records.
