recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crush hazards and struck-by hazards during the operation
of powered industrial
trucks which did not have restraint systems (seatbelts) installed:
a) Facility - On or about 06/20/11 employees operated th Toyota forklift
Model No 42-3FCG15
for transporting materials. The vehicle did not have seatbelts installed.
Abatement method: Among other methods, a feasible and acceptable abatement
method to
correct this hazard is to install and utilize a restraint system in
accordance with the current
version of the American National Standards Institute (ANSI)/Industrial
Truck Standards
Development Foundation (ITSDF) B56.1-2009, "Safety Standard for Low Lift
and High Lift
Trucks).
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

were exposed to struck-by hazards created by the shattering of PVC pipe
under pressure, in
service for distributing compressed air:
a) The employer installed a 135 ft. long, 1.0 in. diameter PVC pipe for
distribution of
compressed air in the plant.
Abatement method: Among other methods, a feasible and acceptable abatement
method to
correct this hazard is to replace the PVC pipe with a type approved for
use with compressed air;
i.e., non-shattering plastics like HDPE and ABS, or metals like copper or
steel.
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
