recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crush hazards and struck-by hazards during the operation
of powered industrial
trucks which did not have restraint systems (seatbelts) installed:
a) Facility - On or about 06/30/11 employees operated the Hyster Model No
S30E (LPG
powered) and the vehicle did not have a seatbelt installed.
a) Throughout the facility - On or about 06/30/11 employees operated the
Clark 10 forklift
(electric). The male part of the latch in the seatbelt was missing and the
seatbelt was disabled.
Abatement method: Among other methods, one feasible and acceptable
abatement method to
correct this hazard is to install and utilize a restraint system in
accordance with the current
version of the American National Standards Institute (ANSI)/Industrial
Truck Standards
Development Foundation (ITSDF) B56.1-2009, "Safety Standard for Low Lift
and High Lift
Trucks".
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
