from the

shelter module were
potentially exposed to toxic hazards without an evaluation of the impact
of toxic material
release prior to occupancy.  This was observed on or about 4/25/2011.

detection system.  This was observed on 4/25/2011.

not informed or trained in the change prior to start-up of the affected
