Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing injuries from falling sections of the building's
facade which were not
properly supported with an adequate shoring system:
a)  Lafayette Building, Kimpton Hotel Monaco Project, 437 Chestnut Street,
Philadelphia, PA 19106-  Demolition of the sidewalk above the basement of
the building
compromised the support of the building's facade.  An inadequate shoring
system lacking
lateral shoring or bracing failed to hold the building's facade in place,
exposing
employees to crushing hazards from the granite base stones loosening and
falling to the
basement work area below.  This condition was noted on or about August 3,
2011.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE
ABATEMENT METHOD TO CORRECT THIS HAZARD IS:  To follow and
implement the applicable sections of ANSI/ASSE (American National Standards
Institute/American Society of Safety Engineers) consensus document

Safety and Health Program Requirements for Demolition Operations- American
National Standard for Construction and Demolition Operations including but
not
limited to:
Section 5 General Protection
Section 5.1 During demolition, inspections by a competent person shall be
made to
detect hazards resulting from weakened or deteriorated floors, walls or
loosened
material.  No worker shall be permitted to work where such hazards exist
until they
are corrected by shoring, bracing, or other effective means.  Access
controls shall
be established to prevent unauthorized entry into the work zone.
