to potential crush injuries from a
falling load with an inadequately engineered lifting devices:
(a)Valco Companies, Safeguard Products, Inc. - Employees used homemade
below-the-hook
lifting devices that had no permanently affixed durable identification
stating the rated capacity,
manufacturer, serial number, lifting device weight, etc., on or about May
