Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an injury from fall and overhead hazards in an unguarded manhole: (a)889 Central Manor Road, Lancaster, PA - Employees were exposed to a seven (7) foot fall hazard when the manhole opening was not barricaded, on or about May 25, 2010. VERIFICATION REQUIRED A feasible method of abatement among other methods would be to meet the requirements of 29 CFR 1910.146(c)(5)(ii)(B). When an entrance cover is removed, the opening shall be promptly guarded by a railing, temporary cover, or other temporary barrier that would prevent an accidental fall through the opening and that would protect each employee working in the space from foreign objects entering the space.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an injury from a potential atmospheric hazards: (a)889 Central Manor Road, Lancaster, PA - Employees were exposed to a potential atmospheric hazard when the air was not tested prior to entering a manhole, on or about May 25, 2010. VERIFICATION REQUIRED One feasible abatement method among others would be to meet the requirements of 29 CFR 1910.146(d)(5)(i) of the permit-required confined space program by testing atmospheric conditions in the permit space to determine if acceptable entry conditions existed before entry was is authorized to begin.
