Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards from welding an additional piece on the hook, thus potentially changing the integrity and strength of the hook: (a)Facility Wide - Employees used a modified hook to lift parts weighing up to 1000 pounds. The employer had a piece welded to the back of the hook, on or about February 8, 2011. ABATEMENT NOTE: Among other methods, feasible and acceptable means to correct the condition include but are not limited to: 1. Replace the hooks with new hooks in accordance with ASME B30.10 Hooks Section 10-1.1.2 Construction. 2. Replace the hooks with a 'below the hook' lifting device that meet the requirements of ASME B30.20, Below the Hook Lifting Devices. VERIFICATION REQUIRED
