a.At the general factory/receiving department, an employee was observed
operating a forklift without wearing a seat belt, which exposed the
employee to caught by/in between and struck by hazards, on or about
8/18/2010.
b.At the shipping department, an employee was observed operating a forklift
without wearing a seat belt, which exposed the employee to caught by/in
between and struck by hazards, on or about 8/18/2010.
Among the feasible and useful methods to correct the hazard is to prohibit
employees to operate this type of equipment without wearing the seat belt.
Note:Certification of abatement must be submitted for this citation item
as required by 29 CFR 1903.19(c).
