Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
On or about May 11, 2011, the employer did not ensure the Western
hydraulic
lift, model R-L-
A, serial number 9521, was properly maintained. The employees were exposed
to crush-by and
struck-by hazards from the lift falling.
Among other methods, the following are feasible and acceptable abatement
methods to correct
the hazards: 1. Institute a preventive maintenance program according to
manufacturer's
specifications. 2. Maintain the automatic mechanical safety device to hold
the lift. 3. Provide
training to employees on the proper use of the auto lift. 4. Ensure lifts
are marked with their
rated load capacity. 5. Display the lift operator instruction in a
conspicuous location.
ANSI/ALIOIM & ANSI B153 describe industry accepted methods to control
hazards related to
auto lifts.
