Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to.
This violation was observed on or about August 24, 2011, on the south side
of 820 and Haltom
Road, in Haltom City, Texas.  Employees were exposed to being struck by
pipe casing and other
pieces of equipment when lifted by a hoist on a Komatsu H2K6T.  The
employer did not ensure
that the hook safety latch was in place in accordance with ASME/ANSI
B30.16 and the
manufacturer operations manual.
Among other feasible methods to correct the identified hazard(s):  (1)
Conduct frequent and
periodic inspection of the hoists in accordance with ASME/ANSI
B30.16-2007, Chapter 16-2
and the manufacturers operation manual.  2)  Inspect, maintain and/or
replace
the hook and/or
hook safety latch to ensure it meets the requirements of the
manufacturer's operation manual,
and ASME/ANSI B30.16-2007, Chapter 16-1.2.9.
