On or about March 17, 2010, an employee climbed up the side and to the top
of a trench box
approximately 15 feet above the ground to secure a wire rope sling to the
lifting points without
fall protection. The trench box was being unloaded by the excavation crew
on site with an
excavator that had a sling attached beneath the bucket with a shackle.
Among others, one feasible and acceptable means of abatement would be to
work from an
extension ladder to attach the sling hooks to the lifting points.

sampling for welding
fumes. B) Respiratory protection. C) Communication equipment. D) Stand-by
person(s) with
readily available rescue equipment. E) A permitting system to assess and
remove the hazards
of the confined space.
Among others, one feasible and useful method of abatement would be the
full implementation
of the elements required by the American National Standards Institute
(ANSI) standards Z117.1
(Safety Requirements for Confined Spaces).
