Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling from elevation: a) On or about 8/25/10 and at times prior there to employee(s) were exposed to a fall approximately 9 ft. 8 in. to the ground level below while transitioning from and to the Werner self supporting ladder, A-Frame, to the top of the concrete cell. AMONG OTHER METHODS, FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT THIS HAZARD ARE TO: PROVIDE AND ENSURE EMPLOYEES ARE USING AN 18 FT. EXTENSION LADDER. IN ADDITION, AN ALTERNATIVE COULD BE TO CONTINUE USING THE EXISTING WERNER SELF SUPPORTING LADDER BY TYING IT OFF TO THE STRUCTURE TO PREVENT SIDEWAYS MOTION AS PER ANSI A14.5-1982 SECTION 9.3.7 CLIMBING LADDERS.
