On or about December 29, 2010, a person riding in the bucket fell out and
was injured when
the vehicle ran him over.

allowing employees to
enter, ride and work from the buckets of front end loaders or similar
equipment as described in
the manufacturer's warnings, and the Equipment Manufacturing Institute
