Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment wich were free from
recognized hazards that
were likely to cause death or serious physical harm to employees in that
employees were
exposed to the hazards of potentially hazardous atmosphere:
a)On or about 12/20/2010, at the work site, the employees were exposed to
potential hazardous atmosphere while entering a 35 foot deep tunnel a
permit
required confined space.  Sperian Biosystems multipro meters, SN 64815 and
63642, and Sperian Cannonball 3 atmospheric monitor SN 05477 were not
calibrated or verification of accuracy were not done daily.
One feasible method of abatement is to obtain a span gas calibration test
kit and to perform
daily checks on any equipment to test each sensor used to verify the
accuracy of the
calibration as recommended by the manufacturer.  Ensure all equipment used
is calibrated
annually by the manufacturer or approved servicing company.
