a)On or about 05/09/11, the employer did not ensure that employees working
with cytotoxic drugs (antineoplastic drugs) such as but not limited to
ifosfamide, cisplatin, bleomycin, vinblastine, and vincristine were
covered by
a comprehensive spill and decontamination program.
AMONG OTHER METHODS, FEASIBLE AND ACCEPTABLE ABATEMENT TO
CORRECT THIS HAZARD IS TO:
1.HAVE THE BIOLOGICAL SAFETY CABINET CERTIFIED EVERY SIX
MONTHS AS RECOMMENDED IN THE ASHP TECHNICAL
ASSISTANCE BULLETIN ON HANDLING CYTOTOXIC AND
HAZARDOUS DRUGS.
2.DEVELOP SPILL AND DECONTAMINATION PROCEDURES SPECIFIC
FOR THE SIZE OF THE SPILL INCLUDING INSTRUCTIONS ON WHAT
PROCEDURES TO TAKE IF OUTSIDE THE BIOLOGICAL SAFETY
CABINET AND THE PERSONAL PROTECTIVE EQUIPMENT
REQUIRED FOR USE DURING CLEAN UP DEPENDING ON THE SPILL
QUANTITY.
3.WHEN POTENTIAL EXPOSURE EXISTS SUCH AS DURING SPILL
CLEAN-UP PROVIDE NIOSH-APPROVED RESPIRATORY PROTECTION
(AT LEAST AN N-100 PARTICULATE RESPIRATOR), AND DEVELOP
AND IMPLEMENT A RESPIRATORY PROTECTION PROGRAM WHICH
COMPLIES WITH 29 CFR 1910.134.
4.DEVELOP AND IMPLEMENT A PROGRAM TO EVALUATE AND
IDENTIFY HAZARDOUS DRUGS AND TRAIN EMPLOYEES ON THE
HAZARDS OF THE DRUGS AND THE METHODS TO BE USED TO
PREVENT OR MINIMIZE EXPOSURE TO THE DRUGS.
5.FOLLOW THE GUIDELINES ESTABLISHED BY THE AMERICAN
SOCIETY OF HOSPITAL PHARMACIST (ASHP), 1990, ASHP
TECHNICAL ASSISTANCE BULLETIN ON HANDLING CYTOTOXIC
AND HAZARDOUS DRUGS, OR THE OSHA TECHNICAL MANUAL
(TED 1.15) SECTION VI: CHAPTER 2 CONTROLLING OCCUPATIONAL
EXPOSURE TO HAZARDOUS DRUGS AND THE NIOSH ALERT :
PREVENTING OCCUPATIONAL EXPOSURES TO ANTINEOPLASTIC
AND OTHER HAZARDOUS DRUGS IN HEALTH CARE SETTINGS.
