recognized hazards that are causing or likely to cause death or serious
physical harm to his
employees in that employees were exposed to struck by and crushing hazards.
(a) On or about 02/11/11, and at times prior there to, employees were
exposed to a struck by
and/or crushing hazard while elevating vehicles on a two post Challenger
lift serial #272014
located in Bay #2 without a functioning stop on the lifting arm extension.
(b) On or about 02/11/11, and at times prior there to, employees were
exposed to a struck by
and/or crushing hazard while elevating vehicles on an in floor Twin lift
serial #7000 located
in Bay #4 without a functioning stop on the lifting arm extension.
Among other methods, one feasible and acceptable method to correct the
hazard noted is to
follow the requirements in ANSI/ALI ALOIM 2008 which include, among other
requirements, the following:
1. Provide effective training to employees on the proper use and care of
auto lifts.
2. Institute a preventive maintenance program according to the
manufacturer's specifications.
3. Schedule and complete all periodic inspections.
