maintenance and repair activities, the employer did not ensure the work
platform on the forks
of the Yale propane fork lift Model # GLC080LFNSAE085, Serial # N5458951,
were secured
to the forks of the forklift and employees used appropriate fall
protection systems/devices.
Abatement Note: One feasible abatement method would be to follow the
precepts of
ANSI/ITSDF B56.1-2009, Safety Standards for Low Lift and High Lift Trucks,
Section
4.17-
Elevating Personnel; paragraph 4.17.3 which requires that platforms be
secured to the forks of
the forklift, the area around the elevated work be coned off, and
personnel are to remain on the
platform floor. Other methods include wood or metal ladders according to
29 CFR 1910.25 or
29 CFR 1910.26 or use of mobile scaffolding according to 29 CFR 1910.28.
In accordance with
29 CFR 1903.19(c), abatement certification is required for this violation
(using the
CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
