where employees were exposed to struck by hazards due to the hose, under
high pressure,
not being secured in such a way as to restrict movement of the hose in the
event of a
rupture.
Among other feasible methods to correct the identified hazard, one method
would be to
secure high pressure hoses in accordance with American Petroleum Institute
Recommended
Practice (API-RP) 54 para.6.3.2 which states " Well operations involving
the rig shall not be
commenced until the rig is rigged up in a safe manner".
