employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to THE HAZARD OF BEING STRUCK-BY:
a)Press Room, on or about 11/23/10:  An overhead hoist support structure
with
a 1-ton Northern Industrial hoist suspended from it, was not marked with a
rated load capacity.
Among other methods, one feasible and acceptable method of abatement is to
inspect
the support structure, determine its rated load capacity and mark the
structure with
that rated load.  Reference ASME B30.11-2010 Monorails and Underhung
Cranes,
Chapter 11-1, Section 11-1.2 Monorails and Chapter 11-2 Inspection and
Testing.
b)Shear Area, on or about 11/23/10:  A single monorail system with a CM
hoist
suspended from it, was not tested by a qualified person or manufacturer
following modifications made by the Employer.
Among other methods, one feasible method of abatement is to have the
monorail
system inspected and tested by a qualified person or manufacturer in
accordance
with ASME B30.11-2010 Monorails and Underhung Cranes, Sections 11-1.3.5 and
11-2.2.
Abatement certification must be submitted for this item.
