Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing hazards:
(a)  On or about May 12, 2010, an employee engaged in "skidding a rig" was
pinned
between an all-terrain forklift and a semi-truck suffering fatal injuries
three miles west
of 76th Avenue on 34th Street, Parshall, ND.
Abatement Note:
Among other methods, some feasible and acceptable methods of abatement
would
include:
1.Develop, document, and enforce a set of drilling rig moving procedures
and train
employees towards those procedures prior to allowing them to execute a
drilling
rig move.
2.Develop and enforce a consistent audible means of communication between
the
vehicle operators and the scampers to prevent misinterpretation.
3.Ensure that scampers and vehicle drivers are trained to a uniform set of
hand
signals to prevent confusion.
