Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:05/24/10 Location:North Side of Building Equipment:JCB 506C Forklift, Model: #5060, Serial: #SLP60600 E0585-85 Condition:An employee, operating a forklift, did not use its equipped seat belt. Among other methods, one feasible and acceptable method to correct this hazard is to utilize a restraint system in accordance with the current version of American National Standards Institute (ANSI) B56.1, "Safety Standard for Powered Industrial Trucks," and American Society of Mechanical Engineers (ASME) B56.1 - 2000, "Safety Standard for Low Lift and High Lift Trucks." Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.

Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:05/24/10 Condition:Employees were exposed to the hazard of being crushed/struck by collapsing roof trusses due to inadequate temporary bracing techniques. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install temporary bracing in accordance with the Wood Truss Council of America recommendations per BCSI-B1. Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.
