did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to: The hazard of being struck by a compressed gas cylinder. This
condition occurred
in numerous instances on or about April 30, 2010 and at times prior
thereto throughout the yard
in areas located outdoors. The cylinders were stored upright but were not
secured with a chain
or strap to prevent them from falling. Secure with a chain or strap to
prevent falling. Among
other methods, one feasible and acceptable abatement methods to correct
this hazard is to act
in accordance with the Compressed Gas Association pamphlet, Safe Handling
of Compressed
Gases in Containers, CGA P-1-2006 paragraph 5.7.4, All compressed gas
cylinders in service
or in storage at user locations shall be secured to prevent falling or
rolling.:
a)Yard In Front of Office - Employees were observed working around 10
unsecured
compressed gas cylinders which were in storage, exposing them to falling
hazards, on or about 04/30/10.
b)Yard at Truck Entrance - Employees were observed working around 5
unsecured
compressed gas cylinders which were in storage exposing them to a falling
hazard, on or about 04/30/10.
c)Yard Between Shear Pump House and Warehouse - Employees were observed
working around 2 unsecured compressed gas cylinders in service for cutting
operations exposing them to a falling hazard, on or about 04/30/10.
d)Yard Next to Bailing Conveyor - Employees were observed working around 2
unsecured compressed gas cylinders in service for cutting operations
exposing
them to a falling hazard, on or about 04/30/10.
e)Yard at Truck Entrance - Employees were observed working around 1
unsecured
compressed gas cylinders in service for cutting operations exposing them
to a
falling hazard, on or about 04/30/10.
