Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to falls from an elevated aerial lift working platform: A.Colebrook River Road Project: The personal fall arrest system used by an employee was not rigged to prevent the employee from free falling no more than 6 feet nor from contacting the ground; B.Colebrook River Road Project: The employer did not provide adequate training done by a competent person for each employee in the calculation of fall clearance distances for personal fall arrest systems.
