Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to fall hazards: 94 Cooper Street, Manchester, CT.: On or about 4-26-11 an employee working aloft topping a tree with a chain saw was exposed to 30 to 35 foot fall hazards by not using a second tie off point. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS TO: Comply with the requirements of ANSI Z133.1-2006 Arboricultural Operations - Safety Requirements Section 6.3.8, use a second means of tie off when using chain saws aloft. Section 8.1.18 states that when aloft a climbing line and a work positioning lanyard shall be used.
