Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to potential fire and explosion hazards: Building B, Mold Maintenance Area: The Maxi-Blast Pressure Blast System Model MB 4B- 4BP-DC200 consisting of a blast cabinet and dust collector. A.Blast Cabinet: Employees were exposed to fire and/or explosion hazards because the Maxi-Blast Pressure Blast System's blast cabinet (where manufacturing molds were blast cleaned by plastic beads in a process that generates combustible plastic dust) produced sparks when the blast nozzle was in use. There was incomplete grounding within the blast cabinet, which enhanced the fire/explosion hazards. In addition, the blast nozzle could operate even when the dust collector was off, which permitted the accumulation of combustible dust in the blast cabinet. Among other methods, one feasible and acceptable abatement method to correct this hazard is: to eliminate ignition sources in the cabinet, to ensure that the blast cabinet is completely grounded, and to interlock the blast nozzle with the dust collector to ensure that the dust is properly evacuated from the blast cabinet. Refer to National Fire Protection Association (NFPA) 654 Standard for the Prevention of Fire and Dust Explosions of Combustible Particulate Solids for further details on feasible abatement methods. B.Dust Collector: Employees were exposed to fire and/or explosion hazards because the dust collector component of the Maxi-Blast Pressure Blast System (which generates combustible plastic dust as it blast-cleans the manufacturing molds) was located inside the building near the blast cabinet in the work area. The dust collector was not equipped with systems for fire/explosion prevention, suppression, venting and/or isolation, to prevent or limit the spread of a potential dust fire/explosion into the general work area and/or back into the blast cabinet. Also, the dust collector was joined to the blast cabinet's exhaust duct with a plastic hose and it was equipped with sateen cotton collection bags, both of which are combustible and both of which lack anti-static properties needed to minimize ignition possibilities. In addition, the dust collector recycled exhaust air back into the work area such that a fire/explosion in the collector could spread inside the building. Among other methods, one feasible and acceptable method to correct this hazard is to: -locate the dust collector outside of the building or equip it with protective devices, such as fire detection, explosion suppression, explosion isolation, and explosion venting; -isolate the dust collector from the blast cabinet to eliminate the propagation of fire/explosion from one to the other; -install metal ducts (noncombustible) instead of plastic hose;-ensure that all components are conductive and completely bonded and grounded. Refer to National Fire Protection Association (NFPA) 654 Standard for the Prevention of Fire and Dust Explosions of Combustible Particulate Solids for further details on feasible abatement methods.
