Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:07/30/10 Location:Outside of the Northwest Corner of the Ladle House, Ambulance Station 105 Equipment:Shop-Made Oxygen Natural Gas Lance Condition:Employees were exposed to burns when the oxygen supply hose detached from the clamped portion of the fitted hose assembly while employees were melting solidified iron from the sub-car. 105 Equipment:Shop-Made Oxygen Natural Gas Lance Condition:Employees were exposed to burns when the oxygen supply hose detached from the clamped portion of the fitted hose assembly while employees were melting solidified iron from the sub-car. Among other methods, one feasible and acceptable abatement method is to insure a tailpiece is part of the clamp assembly as in Compressed Gas Association E-1--2000 Standard Connections for Regulator Outlets, Torches, and Fitted Hose for Welding and Cutting Equipment. Another feasible and acceptable abatement method is to include the stand as in years past, so that the lance does not rest at the hose connection when not held by employees. Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.

Among other methods, one feasible and acceptacle abatement method to correct this hazard is to provide a lance that is an approved as in ANSI Z4.1:2005 Safety in Welding, Cutting and Allied Processes 3.2.1.4 Approved Equipment. Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.
