furnish to each of his employees a place of employment which was free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to an amputation hazard.
The employees were required to operate a press brake without a guard to
cover the foot
pedal used to activate the machine. Among other methods, one feasible and
acceptable
method to correct this hazard is to provide a guard or cover for the foot
pedal that meets
the requirements on ANSI (American National Standards Institute)
B11.3-2002 section
6.10.4 Safety Requirements for Power Press Brakes. Which states: When
provided, foot
controls shall be protected to prevent actuation from falling or moving
objects, or from
unintended actuation.
