furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushed/struck-by hazards:
(a)On or about June 28, 2010 - Welding Department, the hook on the Stahl
hoist did
not have a safety latch installed.
Among other methods, one feasible and acceptable method to correct this
hazard would be to
comply with the requirements of the American Society of Mechanical
Engineers Standard ASME
B30.10-1993, Hooks, Section 10-1.1.2(b).
