29 CFR 1910.119(f)(3): The operating procedures were not reviewed as often as necessary to assure that they reflect current operating practice, including changes that result from changes in process chemicals, technology, and equipment, or changes to facilities: a)In the reduction area: On or about June 7, 2010 and at times prior the employer allowed employees to deviate from the operating procedures for Reactor Assembly (WIR112) in that operators would install the CVD reactor chamber on the base and insert and hand tighten 6 to 8 bolts instead of following the procedure that required all reactor chamber flange bolts to be installed and tightened prior to starting the Reactor Drying step. b)In the reduction area: On or about June 7, 2010 and at times prior the employer allowed employees to deviate from the operating procedures for Reactor Assembly (WIR112) in that operators would install the CVD reactor chamber on the base and insert and hand tighten 6 to 8 bolts instead of following the procedure that required all reactor chamber flange bolts to be installed and tightened prior to removing their required flame retardant clothing and respirators. c)In the reduction area: On or about June 7, 2010 and at times prior the employer allowed employees to deviate from the operating procedures for Reactor Drying and Leak Check (WIR120) in that this step was started before the reactor chamber was securely bolted (all bolts installed and tightened) to the base and all hoses were connected.
