and lift an
employee to obtain the trench box chains from the anchor points on the top
of the 10 ft
high trench box. The employee was exposed to a potential crush and/or fall
hazard.
Violation observed on or about 05/19/10.
Feasible and acceptable methods of abatement include, but are not limited
to the following:
1. Follow the operator's manual safety precautions and warnings
2. Prohibit the use of the front end loader to lift and transport
personnel
