A.R.S. 23-403.A.: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to their employees, in that the employer did not ensure that inspection and maintenance procedures were provided and the equipment was being inspected, maintained and documented as required by the manufacturer and/or in accordance with the American National Standards Institute (ANSI) / American Lift Institute (ALI) Standard ALOIM:2008 (Safety Requirements for Operation, Inspection and Maintenance of Automotive Lifts): (a)Maintenance Shop: The employer did not provide periodic inspection procedures for their employees to use for the Golf Lift Automotive/Golf Equipment Lift, Model GL9PN1GRG, Serial number UAI08G0036. (b)Maintenance Shop: One Golf Lift Automotive/Golf Equipment Lift, Model GL9PN1GRG, Serial number UAI08G0036, was not inspected on a daily and weekly basis. (c)Maintenance Shop: One Golf Lift Automotive/Golf Equipment Lift, Model GL9PN1GRG, Serial number UAI08G0036, was not inspected at least on an annual basis by a Qualified Lift Inspector. (d)Maintenance Shop: The employer did not provide periodic maintenance procedures for their employees to use for the Golf Lift Automotive/Golf Equipment Lift, Model GL9PN1GRG, Serial number UAI08G0036.
