a) At Tip Top Poultry, the employer failed to ensure the mechanical
integrity of process
equipment was maintained. On or about August 25, 2010 a release occurred
at the
facility releasing approximately 16 pounds of ammonia. An isolation valve
on the end of
a solenoid valve failed causing an ammonia release on the liquid ammonia
line.

equipment. On or about August 25, 2010 a release occurred at the facility
releasing
approximately 16 pounds of ammonia. Two workers were severely injured as a
result an
exposure to ammonia. Refrigeration technicians were not utilizing
appropriate hand
protection.
