29 CFR 1910.119(j)(4)(i): Inspections and tests were not performed on process equipment to maintain its mechanical integrity: a) At Tip Top Poultry, the employer failed to ensure the mechanical integrity of process equipment was maintained. On or about August 25, 2010 a release occurred at the facility releasing approximately 16 pounds of ammonia. An isolation valve on the end of a solenoid valve failed causing an ammonia release on the liquid ammonia line. ABATEMENT CERTIFICATION AND DOCUMENTATION NOT REQUIRED

29 CFR 1910.138(a): The employer did not require employees to use appropriate hand protection when employees hands were exposed to hazardous condition(s): a) At Tip Top Poultry, the employer failed to ensure that employees engaged in ammonia refrigeration repairs and maintenance utilize all the appropriate personal protective equipment. On or about August 25, 2010 a release occurred at the facility releasing approximately 16 pounds of ammonia. Two workers were severely injured as a result an exposure to ammonia. Refrigeration technicians were not utilizing appropriate hand protection. ABATEMENT CERTIFICATION AND DOCUMENTATION NOT REQUIRED
