that employees were exposed to crushed-by hazards:
(a)On or about September 8, 2010 - Warehouse, an employee was operating a
forklift without utilizing the restraint device.
Among other methods, one feasible and acceptable method to correct this
hazard is to
ensure that forklift operator(s) utilize the restraint devices provided on
forklift trucks.
