recognized hazards that are causing or likely to cause death or serious
physical harm to his
employees, in that employees were exposed to being struck-by and/or
crushed by an
automobile lift, which was not anchored to the concrete floor.
(a)Lightning Hybrids, Inc. & 319 N. Cleveland Ave., Loveland, CO: On or
before January 13, 2011, the employer did not ensure that employees were
protected from struck-by and/or crushing hazards while elevating trucks
using
the Electro-Hydraulic Four-Post Automobile Lift, 14000 Lb. Model, which
was installed without the required floor anchoring bolts in all four posts
of the
power side assembly to provide lift stability.
(b)The hazard of being struck-by and crushed while elevating trucks using
the
Electro-Hydraulic Four-Post Automobile Lift, 14000 Lb. Model, which was
installed without the required floor anchoring bolts in all four posts of
the
power side assembly to prevent shifting or displacement.
Among other methods, one feasible and acceptable method to correct the
hazards
noted above is to follow the manufacturer's installation requirements on
anchoring the
power side assembly. The manufacture installation instruction shall be the
final
authority in all matters pertaining to the installation of the lift.
Planning considerations
shall place paramount emphasis on the written manufacturer installation
instructions.
Other requirements: 1) ANSI/ALI:2001, which addresses the safety
requirements
for installation of surface mounted automobile lifts, floor anchors, and
service.
Furthermore, a manufacturer Installation Report shall be completed for
each lift that
was installed. The Installation Report shall be maintained by the
installing company
and a copy shall be sent to the lift manufacturer.
2) ANSI/ALI ALOM: 2008, which addresses training, maintenance and
inspection
requirements.  1. Provide training to employees on proper use and care of
the lift.
2. Institute a preventive maintenance program according to the
manufacturer's
specifications.  3. Schedule and complete all periodic and annual
inspections.
