Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an amputation hazard. a) 20 Warrick Ave., Glassboro, NJ 08028 - The employer did not fully develop and implement a safe distance program for the protection of press brake operators. Operators were not required to maintain a minimum four inch distance between their hands and the die of the machine. On or about 07/29/2010. NOTE: One feasible abatement method would be to develop and implement a safe distance program that complies with the requirements of ANSI B11.3-1982 and OSHA Directive CPL 2-1.25. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
