Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hoist load hazard: a) Killing Floor: The hook of the Yale electrical lift hoist was not equipped with a safety latch, on or about 11/23/10. ABATEMENT NOTE: Among other feasible and acceptable methods to correct this hazard is to equip the hoist hook with a safety latch as described in ANSI B30.16-2007, Section 16-1.2.9: a latch shall be provided to bridge the throat opening of the hook and retain, under stack conditions, such items as, but not limited to, slings and chains.
