Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a fire and explosion hazard:
a) Powder Area: Portable Donaldson Torit Cabinet Dust Collectors were
located inside the
building and vented inside of the building. They are used for collecting
dust produced
during milling, blending, sieving and pulverizing of various nutritional
and flavoring
powders, including but not limited to Dong Quai Pure Powder, which was
determined
to be explosive. These dust collectors were used without proper grounding
and did not
have a monitoring device to indicate a pressure drop across the filter
media.  These dust
collectors were not vented to the outside and did not have an explosion
suppression
system, on or about September 28, 2011.
Abatement Note: Among other methods, one feasible and acceptable abatement
method to
correct the hazard is to comply with National Fire Protection Association
(NFPA) 61 "Standard
for the Prevention of Fires and Dust Explosions in Agricultural and Food
Processing
Facilities"(2008).
