Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: a)Top of Roof: A metal extension ladder was not tied to the upper access level before climbing onto or off the ladder at the upper level exposing employees to a fall, on or about 11/01/2011. ABATEMENT NOTE: Among other methods, one feasible and acceptable method to correct this hazard is to have the ladder tied to the top of the roof before climbing onto or off the roof.ing
